Only twice have I yelled at a restaurant...and it wasn't an insult per se.

When I had asked for the check and 30 minutes later it still hadn't arrived, I say very loudly: "HAVE YOU EVER NOTICED THAT THE WAITER THAT WAS HOVERING OVER YOUR TABLE BEFORE YOU WERE READY TO ORDER IS NOWHERE TO BE FOUND WHEN YOU WANT TO PAY THE CHECK?!"

The check normally arrives within 30 seconds after that.
